Use the "Request Form" to the left and provide us with some necessary information. Please remember, as a student, you are not permitted to make your own arrangements with any clinical site. Subsequently, pt Health cannot guarantee an internship match between one of our clinics and a student.

However, once a location request has been submitted and approved pt Health will ensure an application for "Provider Placement" is presented to the Academic Coordinator of Clinical Education at your academic institution.
