Employer Extended Health Plans / Benefits
We bill directly to your extended health plan.
Guelph Medical Place Physiotherapy and Health Centre services are covered by most health care insurance plans. If your insurance company allows direct billing and payment we will look after the billing for you. If your insurance company will not allow direct billing and payment you will pay the clinic and submit to your insurance company, however we will assist with your paperwork to make it easier for you.
Motor Vehicle Accident Coverage
We bill directly to your insurance company for you.
If you have extended health care benefits that cover therapy, auto insurance companies in Ontario require that your claim be placed through your extended health plan first. Any remaining balances for services are then covered by your auto accident insurance plan. On your first visit, you may be asked to provide information about your extend health plan.
If you do not have an extended health care benefits which covers therapy, your treatments may be billed directly through your auto accident insurance company.
Workplace Safety Insurance Board (WSIB)
We bill directly to WSIB for you.
WSIB provides physiotherapy coverage for workers injured on the job. We bill WSIB directly for treatments provided, complete necessary forms, progress reports, treatment extensions and communicate with adjudicators, your workplace and other healthcare professionals involved in your treatment. Our team can help you navigate through any WSIB paperwork that you may be unsure about.
Blue Cross Billing
Guelph Medical Place Physiotherapy and Health Centre will bill Blue Cross directly for physiotherapy and massage therapy treatments for members of

Dana Law-Moric B.Sc(HK,) MPhty, MCPA, Physiotherapist, and Clinic Director
Dana completed her Master’s degree in Physiotherapy studies from the University of Queensland in Australia in 2006. She has been practicing in Guelph for the last 5 years and loves being part of the Dawson Road family health team! Dana enjoys working with all conditions and has vast experience working with Private, third party, WSIB and MVA cases. Currently, pursuing her Certificate in Sports Physiotherapy and working for the last few years with the University of Guelph and Guelph Marlin’s swim teams, she has extensive experience with sport related injuries and returning athletes to sport safely. Dana has also taken a variety of post graduate courses to help provide the most current treatments and techniques to provide you with the best care possible. When not working, Dana enjoys spending time outside with her husband and dog.
Dr. Leanne Bender-Pattinson, Doctor of Naturopathic Medicine
Dr. Leanne attended the University of Western Ontario where she earned an Honours Science Degree in Biology and Environmental Science. She then attended the Canadian College of Naturopathic Medicine in Toronto where she completed a 4 year full-time program in Naturopathic Medicine.
Dr. Leanne thrives in her role at Guelph Physiotherapy, where working in a multi-disciplinary clinic provides the best treatment outcomes to her patients. Working closely with the physiotherapists, Dr. Leanne provides her patients with relief from pain and discomfort through the use of acupuncture, heated compresses, and nutritional support. Her patients appreciate the added benefit of having other concomitant concerns being addressed during their visits as well as working with a skilled, caring and charismatic professional. Dr. Leanne enjoys working with individuals who have hormone- related complaints and cardiovascular disease risk factors, including high cholesterol, high blood pressure and diabetes. She also enjoys helping patients who have difficulties with sleep, weight management, thyroid disorders, depression and anxiety. Dr. Leanne has a special interest in women’s health, including fertility concerns, pregnancy and pre- and post-natal care. He has travelled extensively through Europe and Asia where he was first introduced to alternative therapies such as massage therapy and the benefits and results the bring to wellness and rehabilitation. Proficient with Swedish, deep tissue and lymphatic drainage techniques he is able to treat all manner of ailments. Jason enjoys being part of a multidisciplinary team where all facets of health can be integrated into the clients plan for wellbeing. He currently resides in Elora, and continues to travel when time allows.

Jessica Kuntz completed an honours degree in Biological Science with a major in Human Kinetics from the University of Guelph and has certification from Canadian Fitness Professionals as a Personal Training Specialist. She enjoys being active and motivating others to follow an active lifestyle. Jessica joined Guelph Medical Place and Physiotherapy in April 2012. When not working enjoys being outside with family and friends.
