Whether you require services or products as part of your treatment, we will assist you with direct billing or paperwork completion. At Brant 730 Physiotherapy, we accept a variety of insurance plans and payment types. We currently accept:

Important Coverage Information:
OHIP (Ontario Health Insurance Plan)
We direct bill your OHIP plan.
Government funded physiotherapy treatments are available to those who meet certain criteria. Ontario Ministry of Health uses an “Episode of Care” coverage system that consists of a block of physiotherapy treatments provided to patients that meet the OHIP criteria.
The number, frequency and duration of visits offered during each episode of care must be necessary and reasonable for the treatment of your condition. Your physiotherapist will use their clinical judgement and evidence-based best practices to make that decision. Keep in mind, this program does not cover services aimed to maintain an existing level of function. A new referral from a physician or nurse practitioner is required for each episode of care.
OHIP eligibility criteria:
- Age 65 or over
- Age 19 or younger
- Post Hospital stay (within in the last 12 months)
- On Ontario Disability Support Program (ODSP) or Ontario Works
- Other ODSP or Ontario Works without a health card
Employer Extended Health Plans / Benefits
We direct bill your extended health plan.
Many extended health benefits plans include physiotherapy coverage. Our team is happy to help you navigate your insurance plan coverage. While not necessary, we suggest contacting your plan provider to find out:
- If a doctor’s referral is required in order for treatment to be covered.
- How claims are handled, do they offer direct billing or will you have to complete and submit paperwork.

Blue Cross Billing
We will bill Medavie Blue Cross directly for physiotherapy and massage therapy treatments for members of:
- Royal Canadian Mounted Police (RCMP)
- Department of National Defense (DND)
- Canadian Forces
- Veterans Affairs
For any treatments partially covered, you will be asked to pay the difference at each visit.

Joanne Goldsack - Patient Coordinator
Joanne has been at Brant 730 for 16 years.
She has worked with many different physiotherapists and physiotherapy assistants during her time at the clinic and has a wealth of experience in the field.
Joanne enjoys gardening in her spare time along with fly fishing.
Mary Murray - Patient Coordinator
Marylou joined our team in November, 2016. She has many years of experience in customer service, and office administration.
In her spare time, Marylou likes to entertain and spend time with family and friends.
Boris Rakovac - Physiotherapy Support Personnel
Boris has been working as a Physiotherapy Assistant at ptHealth since 2009, after graduating with Honours from Everest College. Boris helps expand our horizon of care through his global experiences, and is fluent in English, Croatian, and Italian.
He is interested in sports, nature, woodworking, photography, and cars.
George Cardenas - Physiotherapist
George obtained his Masters degree in Physiotherapy from McMaster University after completing a Specialized Honours Bachelors degree in Kinesiology and Health Science from York University. He has an avid interest in the use of acupuncture for the treatment of orthopedic conditions and plans to receive his full certification through the Acupuncture Foundation of Canada Institute. Additionally, he plans to pursue further post-graduate training in the field of manual therapy.
Amy Becke - Physiotherapist
In 1997, Amy graduated from of The University of Western Ontario with a Bachelor of Sciences in Physical Therapy and started her career with pt Heath in one of our clinics in Stoney Creek. After 10 years in Stoney Creek, Amy transferred to Brant 730 Physiotherapy and in 2011 became Clinic Director. During this time, she has taken numerous continuing education courses in Manual therapy and Orthopaedics. Amy enjoys working with a wide age range of clients, from infants to seniors, to promote a healthy, balanced and physically active lifestyle.
Rachel Young - Patient Coordinator
Rachel went to School at Everest Collage, where she took the Medical Office Administrator course. She graduated in 2009.
After graduating she began work at pt Health and has been an important part of the team ever since.
Rachel is passionate about her work and goes above and beyond to help patients and provide them with the best care possible.
Farah Mawani - Physiotherapist
Farah Mawani graduated from McMaster university in 2008. She has experience treating a wide range of orthopedic conditions. Her special interests include acupuncture, manual therapy and sports rehab. Farah believes that the best way to recover from injury is through a combination of active exercise and manual therapy. In her free time, she enjoys playing volleyball, hiking and cycling.
