Important Coverage Information:
Employer Extended Health Plans / Benefits
We direct bill your extended health plan.
Many extended health benefits plans include physiotherapy coverage. Our team is happy to help you navigate your insurance plan coverage. While not necessary, we suggest contacting your plan provider to find out:
- If a doctor’s referral is required in order for treatment to be covered.
- How claims are handled, do they offer direct billing or will you have to complete and submit paperwork.

Motor Vehicle Accident Coverage
We direct bill your auto insurance plan.
If you have extended healthcare benefits that cover therapy, auto insurance companies in Ontario require that your claim be placed through your extended health plan first. Any remaining balances for services are then covered by your auto accident insurance plan. On your first visit, you may be asked to provide information about your extend health plan.
If you do not have an extended healthcare benefits that covers therapy, your treatments may be billed directly through your auto accident insurance company.
Workplace Safety & Insurance Board (WSIB)
We direct bill for Workplace Safety & Insurance Board (WSIB) for you.
If you are injured while at work, you have the right to choose which healthcare provider you get care from. You do not need a doctor’s referral to see a physiotherapist. For more information about your rights as an injured worker please see the Workplace Safety and Insurance Act.
If you sustain a workplace injury, you must contact your workplace immediately to ensure the proper paperwork is completed and that your claim is started.
WSIB provides physiotherapy coverage for workers injured on the job. We bill WSIB directly for treatments provided, complete necessary forms, progress reports, treatment extensions and communicate with adjudicators, your workplace and other healthcare professionals involved in your treatment. Our team can help you navigate through any WSIB paperwork that you may be unsure about.
Blue Cross Billing
We will bill Medavie Blue Cross directly for physiotherapy and massage therapy treatments for members of:
- Royal Canadian Mounted Police (RCMP)
- Department of National Defense (DND)
- Canadian Forces
- Veterans Affairs
For any treatments partially covered, you will be asked to pay the difference at each visit.

Waleed Abdel-Razek - Physiotherapist
Waleed graduated from the University of Toronto in 2010 with a Master’s of Physiotherapy. Since then he has continued his pursuit of lifelong learning through post secondary courses (including acupuncture, vestibular rehab, and TMJ treatment).
In addition to practicing for several years in Ontario he has also worked in clinic management in a variety of capacities including mergers and acquisitions as well as project management.
Having grown up in St. John’s Newfoundland he has spent the past two years in Nova Scotia and New Brunswick to apply his knowledge and skills to the communities and lifestyle he has grown up in.
He is happy to be joining the Caledonia Physiotherapy team!
Michelle Dougherty - Massage Therapist
Michelle has been with Caledonia Physiotherapy since her graduation from The Ontario College of Health and Technology in 2011. Michelle has completed courses in athletic and clinical taping, prenatal massage and sport massage. Michelle is also certified in medical acupuncture as well as Chinese scalp acupuncture. Michelle has worked with the host medical teams for: 2015 Toronto Pan Am Games & 2016 IIHL U-18 Women’s World Championships. Michelle is enthusiastic about working in a rehabilitation setting and helping her clients achieve their goals while continuing her education through relevant therapeutic courses.
Daniel Benic - Physiotherapy Support Personnel
Daniel has been working in physiotherapy since 2004. He started off going to school at Canadian Therapeutic College for Massage Therapy. From there he went to Mohawk for PTA/OTA program. Daniel has worked in OHIP, private, hospitals, wellness centers, sports teams and gym facilities throughout his career and has been with pt Health since September 2015. He enjoys sports like soccer and is an avid referee of the sport
Kristy Howe - Patient Coordinator
Kristy graduated from McMaster University in 2012 with honours degrees in gerontology and sociology. Kristy previously worked in the social service field; mainly treatment programs for youth. Kristy joined our team in 2014 as a patient coordinator and is committed to making your pt Health experience great!
Samantha Hill - Patient Coordinator
Samantha has been working in the administrative field since 2009. After a few years of working she decided to continue her education. Samantha graduated with honours from the health care administration program in May 2017 from Fanshawe College. She joined the pt health Caledonia team in June 2017 as a Patient Coordinator. Samantha is dedicated to making every patient’s visit a great experience!
Kyle Dertinger - Physiotherapist
Hon.B.A., MS.c. (PT)
Kyle started his career with pt Health working as a Kinesiologist in 2009 after graduating with an honours degree in Physical Education and Kinesiology from Wilfrid Laurier University. He furthered his education with a Masters of Science degree in Physiotherapy from McMaster University in 2013. As a new graduate, he is looking forward to continuing his education in manual therapy and acupuncture. Away from the clinic, Kyle is an outdoor enthusiast. He enjoys playing volleyball and soccer.
