To come up with your treatment program, a therapist will gather key information about your condition. Employer Extended Health Plans / Benefits
We direct bill your extended health plan.
Many extended health benefits plans include physiotherapy coverage. Our team is happy to help you navigate your insurance plan coverage. While not necessary, we suggest contacting your plan provider to find out:
- If a doctor’s referral is required in order for treatment to be covered.
- How claims are handled, do they offer direct billing or will you have to complete and submit paperwork.

Motor Vehicle Accident Coverage
We direct bill your auto insurance plan.
If you have extended healthcare benefits that cover therapy, auto insurance companies in Ontario require that your claim be placed through your extended health plan first. Any remaining balances for services are then covered by your auto accident insurance plan. On your first visit, you may be asked to provide information about your extend health plan.
If you do not have an extended healthcare benefits that covers therapy, your treatments may be billed directly through your auto accident insurance company.
Workplace Safety & Insurance Board (WSIB)
We direct bill for Workplace Safety & Insurance Board (WSIB) for you.
If you are injured while at work, you have the right to choose which healthcare provider you get care from. You do not need a doctor’s referral to see a physiotherapist. For more information about your rights as an injured worker please see the Workplace Safety and Insurance Act.
If you sustain a workplace injury, you must contact your workplace immediately to ensure the proper paperwork is completed and that your claim is started.
WSIB provides physiotherapy coverage for workers injured on the job. We bill WSIB directly for treatments provided, complete necessary forms, progress reports, treatment extensions and communicate with adjudicators, your workplace and other healthcare professionals involved in your treatment. Our team can help you navigate through any WSIB paperwork that you may be unsure about.
Blue Cross Billing
We will bill Medavie Blue Cross directly for physiotherapy and massage therapy treatments for members of:
- Royal Canadian Mounted Police (RCMP)
- Department of National Defense (DND)
- Canadian Forces
- Veterans Affairs
For any treatments partially covered, you will be asked to pay the difference at each visit.

Candice Yundt - Patient Coordinator
Candice was born and raised in Stratford and has lived in Tavistock for 11 years. She has worked as a Patient Coordinator, at Tavistock Physiotherapy Centre, for the past 3 years.
When not laughing and joking with patients and staff, Candice loves spending her time with her daughter and son and being a hockey mom. She also loves spending time with friend and loved ones, spending some of her free time traveling, doing photography and volunteering when she can.
Barbara Neufeld - Massage Therapist
Barb Neufeld was born and raised in St.Marys. She graduated from Medix College in London and passed her provincial exams in 2011 to become Registered in Massage Therapy.
Barb has attended multiple workshops and courses since graduating and continues to expand her education in modalities relating to massage.
Most recently, she has taken courses in Integrated Manual Therapy. This course uses techniques to help realign posture and especially dealing with shoulder imbalances.
Barb can assess certain issues and provide clients with hands on soft tissue work as well as giving clients stretching and strengthening home care to help the client reach the maximum result of postural balance and decreasing common issues, such as; headaches, tension, flexibility issues and general aches and pains.
Massage Therapy is a great preventative measure, as well as, a part of a general health and wellness plan. It is also a way to help manage stress and anxiety.
Victoria Dears - Patient Coordinator
I graduated from the University of Waterloo in 2011 with a Bachelor of Arts in Social Development Studies. I work as a part-time Patient Coordinator with the Stratford ptHealth crew and enjoy working with a positive and inspiring team of people.
Cathrin Winhold - Patient Coordinator
Cathy has over 30 years experience in administration in various non profit and government organizations. As Special Assistant to a very dedicated MPP of Perth-Wellington, Cathy was responsible for communications including news releases and media advisories as well as stakeholder relations. Representing the MPP when he was not able kept Cathy busy attending meetings, and speaking at various openings and events throughout two counties.
Cathy loves to read and to travel and keeps busy playing tennis, caring for sick friends and elderly relatives and working out at the local Goodlife. As a Board Member of Off the Wall Stratford Artist Alliance Cathy helps with fundraising, events and as a community liaison. She enjoys living in the small Hamlet of Sebringville where she loves walking, biking and cross country skiing.
Alison Gray - Physiotherapist
Alison graduated from McMaster University with an M.A. in 1997 and a BHSc. (Physiotherapy) in 1999. She has lived and worked in Stratford, ON, since 2005. She is currently the Clinic Team Manager of Stratford and Tavistock Physiotherapy Centres.
Alison is a Credentialed McKenzie Therapist and Certified by the Acupuncture Foundation of Canada.
Alison has focused her training on pelvic floor rehabilitation, pelvic pain, pre and postnatal physiotherapy and continence training. She has completed Level 3 and A.C.E. courses with Pelvic Health Solutions, The Hypopressive Method, level 2 certification, and The Rost approach to pelvic girdle and low back pain in pregnancy. She recently completed Bulletproof Your Core with Antony Lo to aid women in fitness, crossfit and HIIT programs with their pelvic floors.
In her spare time Alison enjoys spending time with her two small boys, teaching fitness classes, gardening and attempting to avoid stepping on LEGO.
Hans Dijk - Physiotherapist
Hans graduated in 1984 from Academy voor Physiotherapy, Groningen ,The Netherlands.
He started his Physio career at Stratford General Hospital, where he worked for three and a half years. In 1990 he opened Stratford Physiotherapy Centre, where he continues to practice and share his expertise, after selling to PTHealth.
Hans is well known in Stratford and Tavistock. He has extensive experience in orthopedic work and sports injuries, he has a special interest in neuro rehab such as stroke and traumatic brain injuries.
Hans is a certified MacKenzie therapist, specialized in mechanical diagnosis and treatment of back and neck conditions. He continues to be active in many sports and referee multiple levels of soccer in the Stratford area.
Holly Bell - Massage Therapist
Holly Bell has been a registered massage therapist since 2006. She specializes in deep tissue massage, chronic pain conditions and acupuncture. Holly is highly skilled and was asked to teach Massage therapy through her college. She has been increasing her time as an instructor over the last few years, and is highly thought of as an educator in her field.
