Important Coverage Information:
OHIP (Ontario Health Insurance Plan)
We direct bill your OHIP plan.
Government funded physiotherapy treatments are available to those who meet certain criteria. Ontario Ministry of Health uses an “Episode of Care” coverage system that consists of a block of physiotherapy treatments provided to patients that meet the OHIP criteria.
The number, frequency and duration of visits offered during each episode of care must be necessary and reasonable for the treatment of your condition. Your physiotherapist will use their clinical judgement and evidence-based best practices to make that decision. Keep in mind, this program does not cover services aimed to maintain an existing level of function. A new referral from a physician or nurse practitioner is required for each episode of care.
OHIP eligibility criteria:
- Age 65 or over
- Age 19 or younger
- Post Hospital stay (within in the last 12 months)
- On Ontario Disability Support Program (ODSP) or Ontario Works
- Other ODSP or Ontario Works without a health card
Employer Extended Health Plans / Benefits
We direct bill your extended health plan.
Many extended health benefits plans include physiotherapy coverage. Our team is happy to help you navigate your insurance plan coverage. While not necessary, we suggest contacting your plan provider to find out:
- If a doctor’s referral is required in order for treatment to be covered.
- How claims are handled, do they offer direct billing or will you have to complete and submit paperwork.

Motor Vehicle Accident Coverage
We direct bill your auto insurance plan.
If you have extended healthcare benefits that cover therapy, auto insurance companies in Ontario require that your claim be placed through your extended health plan first. Any remaining balances for services are then covered by your auto accident insurance plan. On your first visit, you may be asked to provide information about your extend health plan.
If you do not have an extended healthcare benefits that covers therapy, your treatments may be billed directly through your auto accident insurance company.
Workplace Safety & Insurance Board (WSIB)
We direct bill for Workplace Safety & Insurance Board (WSIB) for you.
If you are injured while at work, you have the right to choose which healthcare provider you get care from. You do not need a doctor’s referral to see a physiotherapist. For more information about your rights as an injured worker please see the Workplace Safety and Insurance Act.
If you sustain a workplace injury, you must contact your workplace immediately to ensure the proper paperwork is completed and that your claim is started.
WSIB provides physiotherapy coverage for workers injured on the job. We bill WSIB directly for treatments provided, complete necessary forms, progress reports, treatment extensions and communicate with adjudicators, your workplace and other healthcare professionals involved in your treatment. Our team can help you navigate through any WSIB paperwork that you may be unsure about.
Blue Cross Billing
We will bill Medavie Blue Cross directly for physiotherapy and massage therapy treatments for members of:
- Royal Canadian Mounted Police (RCMP)
- Department of National Defense (DND)
- Canadian Forces
- Veterans Affairs
For any treatments partially covered, you will be asked to pay the difference at each visit.

Rhandi Brooks - Patient Coordinator
In 2012, Rhandi received a BA in Psychology from The University of Western Ontario. During her Post-Secondary education Rhandi worked as office administrator for a Family Practice & Mental Health Counselor. After graduating in 2012, looking for a new challenge, this led Rhandi to her current role as Patient Care Coordinator at Victoria Community Physical Rehab. She is likely to be the first to greet you when you walk in and her ability to multitask ensures that everything in the clinic runs as smoothly as possible. Rhandi’s favorite part of her job is meeting new faces everyday and making sure every appointment is enjoyable for the patients!
Carissa Williams - Patient Coordinator
Carissa has been working for pt Health since 2012 and is currently the part time patient coordinator at Limeridge Physiotherapy. She has taken related courses in Medical Terminology to better her skills while working in the clinic. She enjoys meeting new faces and getting to know the patients as they regularly attend their visits.
Chelsea Georgeoff - Physiotherapy Support Personnel
Chelsea graduated from the Occupational and Physiotherapist Assistant program at Mohawk in 2010 where she worked in a variety of hospital and clinic environments. She also is a recent graduate of the Kinesiology program at Brock University with hopes of one day becoming a Physiotherapist.
Kapilsinh Rajput - Physiotherapist
Kapil Rajput is an internationally Graduated Physiotherapist since 2008. Kapil Rajput is a Registered Physiotherapist with experience of over 7 years. He Graduated in 2008 with Bachelor of Science in physiotherapy and has practiced variety of different hospitals and clinics. He has finished Diploma in Fitness and Health Promotion Program from Humber College which gave him thorough knowledge about Sports injuries and Athletic training for different sports. He has worked with client of all ages, in private as well as OHIP funded clinics. Kapil has experience in treating Orthopedic Injuries (Fractures, Post op rehabilitation, Arthritis, sports injuries), Neurological conditions as well as Injuries related to motor vehicle accidents and workplace injuries.
Kapil prides himself on his problem solving abilities and thorough assessment techniques. With gentle persistence, Kapil encourage his patient to take a proactive role in their health and rehabilitation.
Dr. John Misale - Chiropractor
Dr. John P. Misale has been a long standing Chiropractor in Hamilton’s North end community. He is a Graduate of McMaster University in the field of Biology ( 1989 ) and of the Canadian Memorial Chiropractic College (1993 ). His practice is geared towards relief of musculoskeletal injuries. He is certified in acupuncture by the International College of Chinese Medicine. He has a long standing relationship with The Hamilton Foundation a community support organization involved with various charities and scholarships initiatives for the residence of Hamilton.
