Evaluating Your Condition
To come up with your treatment program, a therapist will gather key information about your condition. Your answers along with the results of an expert examination will guide us in creating a specific type of treatment program to effectively address your specific condition and eventually, to facilitate your healing.

Important Coverage Information:
OHIP (Ontario Health Insurance Plan)
We direct bill your OHIP plan.
Government funded physiotherapy treatments are available to those who meet certain criteria. Ontario Ministry of Health uses an “Episode of Care” coverage system that consists of a block of physiotherapy treatments provided to patients that meet the OHIP criteria.
The number, frequency and duration of visits offered during each episode of care must be necessary and reasonable for the treatment of your condition. Your physiotherapist will use their clinical judgement and evidence-based best practices to make that decision. Keep in mind, this program does not cover services aimed to maintain an existing level of function. A new referral from a physician or nurse practitioner is required for each episode of care.
OHIP eligibility criteria:
- Age 65 or over
- Age 19 or younger
- Post Hospital stay (within in the last 12 months)
- On Ontario Disability Support Program (ODSP) or Ontario Works
- Other ODSP or Ontario Works without a health card
Employer Extended Health Plans / Benefits
We direct bill your extended health plan.
Many extended health benefits plans include physiotherapy coverage. Our team is happy to help you navigate your insurance plan coverage. While not necessary, we suggest contacting your plan provider to find out:
- If a doctor’s referral is required in order for treatment to be covered.
- How claims are handled, do they offer direct billing or will you have to complete and submit paperwork.

Motor Vehicle Accident Coverage
We direct bill your auto insurance plan.
If you have extended healthcare benefits that cover therapy, auto insurance companies in Ontario require that your claim be placed through your extended health plan first. Any remaining balances for services are then covered by your auto accident insurance plan. On your first visit, you may be asked to provide information about your extend health plan.
If you do not have an extended healthcare benefits that covers therapy, your treatments may be billed directly through your auto accident insurance company.
Workplace Safety & Insurance Board (WSIB)
We direct bill for Workplace Safety & Insurance Board (WSIB) for you.
If you are injured while at work, you have the right to choose which healthcare provider you get care from. You do not need a doctor’s referral to see a physiotherapist. For more information about your rights as an injured worker please see the Workplace Safety and Insurance Act.
If you sustain a workplace injury, you must contact your workplace immediately to ensure the proper paperwork is completed and that your claim is started.
WSIB provides physiotherapy coverage for workers injured on the job. We bill WSIB directly for treatments provided, complete necessary forms, progress reports, treatment extensions and communicate with adjudicators, your workplace and other healthcare professionals involved in your treatment. Our team can help you navigate through any WSIB paperwork that you may be unsure about.
Blue Cross Billing
We will bill Medavie Blue Cross directly for physiotherapy and massage therapy treatments for members of:
- Royal Canadian Mounted Police (RCMP)
- Department of National Defense (DND)
- Canadian Forces
- Veterans Affairs
For any treatments partially covered, you will be asked to pay the difference at each visit.

Michele Clement - Physiotherapy Support Personnel
Physio Support Personnel
Michèle Clément graduated from the University of Ottawa in 2000 with two Bachelor degrees in Social Sciences (Sociology and Leisure Studies). She completed her PTA course in 2001 and has over 14 years of clinical experience. Michèle is fluent in French and English. She is qualified in Medical Compression Therapy and has taken numerous courses on Custom Orthotic Therpapy and Gait Analysis.
Jason Laprade - Massage Therapist
Jason is a local boy, that has lived, played and worked in Cornwall his whole life. He graduated from Elegance College of Massage Therapy in 2014. He has worked in the plant environment, at call centers and loves to engage in sporting activities, so he knows the wear and tear that a body goes through, which sparked his interest in massage therapy. Jason has worked with all kinds of conditions and loves to see the step by step progression as the body heals. Jason will work with his clients to determine the best course of action to get them feeling better as soon as possible. Whether your interest is in rehabilitation or relaxation his repertoire of skills and techniques will help your reach your health goals.
Dr. Chelsea Grant - Naturopathic Doctor
Dr. Chelsea Grant grew up in Cornwall and graduated from St. Joseph’s Secondary School, where she received a Bilingual Diploma, became a St. Joseph’s Club member, and was awarded The Student Council Award, French Award and Religion Award. She then moved to Guelph to obtain an honours degree in Molecular Biology and Genetics at the University of Guelph. It was her time spent training and working as a lifeguard and instructor in the Cornwall area that lead Dr. Grant to choose a career in the medical field. She had wonderful opportunities to shadow established health care professionals (NDs, MDs, DCs, and Midwives) that helped shape her view of medicine. Dr. Grant is licensed and registered with the Board of Directors of Drugless Therapy Naturopathy (BDDT-N). She is a member of the Canadian Association of Naturopathic Doctors (CAND) and the Ontario Association of Naturopathic Doctors (OAND).
Dr. Julia Esposito - Naturopathic Doctor
Dr. Julia Esposito, ND grew up in Cornwall and graduated from St. Joseph’s Secondary School before attending Mount Allison University in Sackville, New Brunswick. While at university she played varsity soccer and hockey, skills she honed playing Blazers soccer and Typhoons hockey growing up in Cornwall.
She obtained her Bachelor of Science degree in Biology and Psychology and the following year moved to Toronto to attend the Canadian College of Naturopathic Medicine (CCNM).
Upon graduating from the CCNM, Dr. Esposito returned to her home town to set up her general family practice. Dr. Esposito has a special interest in anti-aging medicine, women’s health, cancer therapies and chronic pain management. She is also certified in cosmetic acupuncture, mesotherapy and intravenous therapy.
Lauren Beach - Physiotherapy Support Personnel
Lauren had always been a part of the high school sport teams that helped her in making a career decision. She studied Human Kinetics at the University of Ottawa and received her Bachelor of Science and Honours in human kinetics. She likes spending time learning about exercise physiology to help guide her patients through their rehabilitation. In her spare time she enjoys being at the gym, hiking, walking her dogs and being with family. She recently received a Nutrition and Wellness coach certificate to discover on how to make healthier lifestyle decisions for herself and to help guide others.
Dr. Anastasia D’Annibale - Naturopathic Doctor
Dr. Anastasia D’Annibale, ND grew up in Cornwall and is happy to return to the area and improve the health & wellness of her community. She completed a Bachelor of Health Sciences honours degree from Western University. Her studies focused on analyzing health care models, health policies, and health promotion programs. She gained a thorough understanding of the gaps in our health care system and the need for an integrative and preventative approach to chronic health concerns. This drive to be part of the change, led her to continuing her education in naturopathic medicine at the Canadian College of Naturopathic Medicine in Toronto. During her clinical year, she provided naturopathic care to patients at the Robert Schad Naturopathic Clinic. She had the opportunity to treat a wide range of health concerns as well as to be part of a group of interns that focused on the treatment of reproductive health concerns and infertility. Being passionate about providing access to health care in under-serviced communities locally and abroad, she completed clinical rotations with Lakeshore Area Multi-Service Project at a community health centre in Toronto, with Natural Doctors International at a clinic on Ometepe Island in Nicaragua, and with Naturopaths Without Borders at various pop-up clinics near Cap-Haitien, Haiti.
Dr. D’Annibale is licensed and registered with the College of Naturopaths of Ontario and is a member of the Canadian Association of Naturopathic Doctors. She has a general family practice with special interests in women’s health, fertility, pain management, digestive health, and healthy aging. She is currently accepting new patients.
Kayla McDougall - Physiotherapist
BScHKin, MScPT, PgCStroke
Kayla grew up in Cornwall and graduated from École Secondaire La Citadelle. She played competitive softball and basketball in the area where she developed an interest in training and rehabilitation. She completed her Honours Bachelor’s of Science in Kinesiology at Queen’s University in Kingston and worked as a personal trainer while completing her degree. She also continued playing softball for the Queen’s Women’s varsity team. Kayla completed her Master’s degree in Physiotherapy at the University of Ottawa, where she gained clinical experience in a variety of settings. Kayla has completed continuing education courses for Acupuncture, Concussion Rehabilitation, TMJ treatments and Vestibular conditions. She has completed her post-graduate certificate in Stroke Rehabilitation through the University of Alberta. Kayla recently completed training on treating mastitis and inflammatory breast conditions during lactation.
