Employer Extended Health Plans / Benefits
We direct bill your extended health plan.
Many extended health benefits plans include physiotherapy coverage. Our team is happy to help you navigate your insurance plan coverage. While not necessary, we suggest contacting your plan provider to find out:
- If a doctor’s referral is required in order for treatment to be covered.
- How claims are handled, do they offer direct billing or will you have to complete and submit paperwork.

Motor Vehicle Accident Coverage
We direct bill your auto insurance plan.
If you have extended healthcare benefits that cover therapy, auto insurance companies in Ontario require that your claim be placed through your extended health plan first. Any remaining balances for services are then covered by your auto accident insurance plan. On your first visit, you may be asked to provide information about your extend health plan.
If you do not have an extended healthcare benefits that covers therapy, your treatments may be billed directly through your auto accident insurance company.
Workplace Safety & Insurance Board (WSIB)
We direct bill for Workplace Safety & Insurance Board (WSIB) for you.
If you are injured while at work, you have the right to choose which healthcare provider you get care from. You do not need a doctor’s referral to see a physiotherapist. For more information about your rights as an injured worker please see the Workplace Safety and Insurance Act.
If you sustain a workplace injury, you must contact your workplace immediately to ensure the proper paperwork is completed and that your claim is started.
WSIB provides physiotherapy coverage for workers injured on the job. We bill WSIB directly for treatments provided, complete necessary forms, progress reports, treatment extensions and communicate with adjudicators, your workplace and other healthcare professionals involved in your treatment. Our team can help you navigate through any WSIB paperwork that you may be unsure about.
Blue Cross Billing
We will bill Medavie Blue Cross directly for physiotherapy and massage therapy treatments for members of:
- Royal Canadian Mounted Police (RCMP)
- Department of National Defense (DND)
- Canadian Forces
- Veterans Affairs
For any treatments partially covered, you will be asked to pay the difference at each visit.

Don Lesar - Physiotherapist
HBSc, MSc PT, FCAMPT
Don completed his Masters of Science in Physiotherapy at McMaster University. Prior to Physiotherapy, Don completed a four-year undergraduate degree in Health Studies and worked as a strength and conditioning coach throughout his degree. He has taken additional courses in manual therapy, acupuncture, soft tissue release and therapeutic taping. Don successfully completed the Advanced Orthopaedic Division exam in Fall 2015 conferring him the FCAMPT designation. Don’s special interests include return to sport and return to work conditioning; however, he takes pride in helping people of all ages reach their functional goals.
Stephanie Aubin - Physiotherapist
Stephanie is a graduate of the University of Ottawa, where she completed an Honor?s Bachelor in Health Science, and a graduate of Queen?s University where she completed a Master?s of Science in Physiotherapy. Stephanie has had the opportunity to work with both inpatients and outpatients allowing her to become a well-rounded practitioner. Stephanie utilizes manual therapy, exercise prescription, athletic taping & kinesio taping, soft tissue techniques and modalities as part of her treatment. Stephanie has had the opportunity to work with people of all ages, including persons recovering from joint replacement surgery, athletes following injury, and persons following stroke. The most rewarding part of her job is giving you the tools you need to get back to your full function.
Julia Clark - Physiotherapy Support Personnel

Julia graduated in 1996 with a diploma in Sports Injury Management and a degree from McMaster University. Since then she has completed a wide variety of continuing education courses. Julia’s commitment and enjoyment in helping clients reach their rehabilitation goals is shown through her long time employment with PT Health.
Michelle Havens - Physiotherapy Support Personnel
Michelle Havens graduated from The University of Western Ontario in 2008 where she completed an Honors Bachelor of Arts, majoring in Kinesiology with a minor in Biology. Since graduating, she has completed courses in orthotic fitting, custom knee brace fitting, compression hosiery, and athletic taping. She has worked at clinics specializing in post-operative hips, knees, and shoulders, where she had a special focus in custom bracing. Michelle has been a Physiotherpy Support Personnel with ptHealth since graduating from university and looks forward to advancing her career and education by taking more specialized courses.
Joanna Scarlett - Physiotherapy Support Personnel
Joanna is a Certified Polestar Pilates Instructor with 4 years experience teaching in a sports and rehabilitation setting. Joanna has grown up teaching children’s dance from the age of 12 and started her
fitness career 6 years ago when she certified as a CanFitPro Personal Trainer. Joanna has taught many different types of fitness such as Senior Fall Prevention Classes, Aqua Fitness, Boot Camps, Zumba and most recently certified as a Total Barre Instructor through Stott Pilates.
The goals she works to achieve with her clients are to build body awareness, optimal posture, create stability, core control and ease of movement.
Jessica McKnight - Massage Therapist
Jess graduated from Medix College in 2010 (obtaining a Massage Therapy Diploma), and is in good standing with the College of Massage Therapists of Ontario. She has been trained in Swedish, deep tissue, and TMJ massage; myofascial release, trigger point therapy, and active release techniques; as well as manual lymph drainage and joint mobilization. Jess plans to further her knowledge and skills by taking additional courses, to continue to help her clients integrate and sustain healthy, functional lifestyles.
Dr. John Malatesta - Chiropractor
Bsc., DC, FCCPOR(C)
John Malatesta graduated from the National University of Health Sciences in January 2000. In 2006 he completed his fellowship in Chiropractic Rehabilitation at the Canadian Memorial Chiropractic College. The following year he attained status as an Independent Chiropractic Examiner. He has also had training in gait analysis and fabrication of custom made orthotics. His approach to treatment includes evidence based manual therapies utilizing manipulation, mobilization and soft tissue treatments, as well as rehabilitative exercises with a focus on return to function. John?s belief is that the patient is an equal partner in their recovery and return to normal function and puts a strong emphasis on education as part of his treatment plan.
Dr. Kandice Tota Vandepoele - Chiropractor
Dr. Kandice Tota obtained her chiropractic degree from Canadian Memorial Chiropractic College in 2010, as well as an Honours Bachelor of Science degree in Biology and Psychology from McMaster University in 2006. She practices a variety of techniques including spinal manipulative therapy, myofascial release therapy, and laser therapy. She is also trained in orthotics fitting, compression stocking fitting, and custom knee bracing.
