Evaluating Your Condition
To come up with your treatment program, a therapist will gather key information about your condition. Your answers along with the results of an expert examination will guide us in creating a specific type of treatment program to effectively address your specific condition and eventually, to facilitate your healing.

Important Coverage Information:
OHIP (Ontario Health Insurance Plan)
We direct bill your OHIP plan.
Government funded physiotherapy treatments are available to those who meet certain criteria. Ontario Ministry of Health uses an “Episode of Care” coverage system that consists of a block of physiotherapy treatments provided to patients that meet the OHIP criteria.
The number, frequency and duration of visits offered during each episode of care must be necessary and reasonable for the treatment of your condition. Your physiotherapist will use their clinical judgement and evidence-based best practices to make that decision. Keep in mind, this program does not cover services aimed to maintain an existing level of function. A new referral from a physician or nurse practitioner is required for each episode of care.
OHIP eligibility criteria:
- Age 65 or over
- Age 19 or younger
- Post Hospital stay (within in the last 12 months)
- On Ontario Disability Support Program (ODSP) or Ontario Works
- Other ODSP or Ontario Works without a health card
Employer Extended Health Plans / Benefits
We direct bill your extended health plan.
Many extended health benefits plans include physiotherapy coverage. Our team is happy to help you navigate your insurance plan coverage. While not necessary, we suggest contacting your plan provider to find out:
- If a doctor’s referral is required in order for treatment to be covered.
- How claims are handled, do they offer direct billing or will you have to complete and submit paperwork.

Motor Vehicle Accident Coverage
We direct bill your auto insurance plan.
If you have extended healthcare benefits that cover therapy, auto insurance companies in Ontario require that your claim be placed through your extended health plan first. Any remaining balances for services are then covered by your auto accident insurance plan. On your first visit, you may be asked to provide information about your extend health plan.
If you do not have an extended healthcare benefits that covers therapy, your treatments may be billed directly through your auto accident insurance company.
Workplace Safety & Insurance Board (WSIB)
We direct bill for Workplace Safety & Insurance Board (WSIB) for you.
If you are injured while at work, you have the right to choose which healthcare provider you get care from. You do not need a doctor’s referral to see a physiotherapist. For more information about your rights as an injured worker please see the Workplace Safety and Insurance Act.
If you sustain a workplace injury, you must contact your workplace immediately to ensure the proper paperwork is completed and that your claim is started.
WSIB provides physiotherapy coverage for workers injured on the job. We bill WSIB directly for treatments provided, complete necessary forms, progress reports, treatment extensions and communicate with adjudicators, your workplace and other healthcare professionals involved in your treatment. Our team can help you navigate through any WSIB paperwork that you may be unsure about.
Blue Cross Billing
We will bill Medavie Blue Cross directly for physiotherapy and massage therapy treatments for members of:
- Royal Canadian Mounted Police (RCMP)
- Department of National Defense (DND)
- Canadian Forces
- Veterans Affairs
For any treatments partially covered, you will be asked to pay the difference at each visit.

Brad Clarke - Physiotherapist
Brad graduated from the University of Toronto in 1994 with a B.Sc.PT. He has had the pleasure of working in many areas of Physiotherapy including many Hospitals, Private Clinics and Home Care. Over the past 15 years he has focused his attention to Outpatient Orthopaedics. Brad has completed many Manual Therapy Orthopaedic Courses, and would like to learn more about Concussions and Concussion Therapy in the future. He loves sports and has a wide range of experience playing and coaching.
Rijul Sharma - Physiotherapist
Rijul believes in diagnosing and treating the root of the problem rather than just the problem itself. After completing four years of bachelor’s degree in physiotherapy from India, he has worked in various hospitals and clinics as a physiotherapist. The focus of Rijul’s practice is on client centred approach with implementation of hands on manual therapy techniques, therapeutic exercise and patient education. He has completed post-graduate training in acupuncture, soft tissue release, kinesio taping and is pursuing further training in various other courses to enhance his treatment skills.
Matthew DeBoersap - Physiotherapy Support Personnel
Matt is a physiotherapy support personnel from Grimsby, ON. Matt graduated with honours from Brock University in the spring 2015 with a Bachelor of Kinesiology and has been working at pt Health ever since. Matt is currently continuing his education at the Canadian Academy of Osteopathy which he started in September 2016.
Matt strives to be open and honest with treatment with a strong emphasis on patient education and rational thinking.
Matt professes not to have any spare time with work and school but he can be found frequenting the local soccer fields or with his friends and family.
Mariam Korea - Physiotherapy Support Personnel
Mariam recently joined the team at Stoney Creek Physiotherapy in April 2016. She graduated with a degree in Honours Kinesiology from McMaster University. Mariam is working towards becoming a Registered Kinesiologist and Clinical Exercise Physiologist. Mariam has a variety of experience working with outpatients in rehabilitation settings and enjoys particularly working with seniors and helping them to improve their quality of life. In her spare time, Mariam enjoys playing the guitar, singing, rock climbing, hiking and exploring different trails in the city.
Katharine Merry - Patient Coordinator
Katie moved here from the UK in 2007 after working 25 years for Her Majesty Customs & Excise as an Executive Officer. She joined the team at Flamborough Physiotherapy as a Patient Coordinator in April 2016.
Her interests include cooking, wine tasting and the love of Zumba.
Karen Parker - Massage Therapist
Karen joined pt Health Flamborough in January 2016 after graduating with honours from Trillium College. Massage Therapy is a second career for Karen. After graduating with a B.Sc. from McMaster University, she worked in the pharmaceutical industry for many years. Massage therapy allows Karen to combine two passions – how the human body works and helping people. Karen loves to see her clients get better – whether it’s recovering from an injury or improving their health. Education is important to Karen and she takes the time to ensure that you understand your condition and shows you simple things to do at home that don’t require fancy equipment. Karen lives in Millgrove with her husband, daughter, dog, cat and several chickens.
Alison Pretty - Massage Therapist
Alison comes to us from Everest College where she received her diploma for Registered Massage Therapy. Prior to choosing massage as her carrier Alison was interested in the mental aspects of personal health having completed many courses in psychology. She believes health to be a physical, mental and spiritual journey and is happy to have the opportunity to help others transform themselves back to health. Alison loves waterfalls, nature, hiking and spends most of her down time with her family, two daughters and Malimute.
Petar Bozic - Massage Therapist
Petar (Pete) comes to pt Health with a fusion of Eastern and Western style massage techniques. Always innovating and researching new ways to effectively treat clients and help them reach their goals. Whether it’s enhancing a competitive athlete, unwinding a working class hero, or helping a new mom be at her best. Pete is honest, positive and witty, which can help you get through the intense parts of a massage treatment, but also let you drift off to that special place during relaxing massage. No passport required.
Katie Dawson - Physiotherapist
Katie received her Master’s Degree in Physical Therapy from the University of Toronto in 2012. She has worked in Waterdown for the past 5 years and is now splitting her treating time between Waterdown and Hamilton. She is currently the Clinic Team Manager of Flamborough and Mountain Physiotherapy and Rehabilitation.
Previously, Katie graduated with an Honours Degree in Kinesiology from Wilfrid Laurier University in 2010. Katie is eager to learn and is taking additional post graduate training in order to further her clinical knowledge. She also is a competitive athlete with an extensive background in ringette. She enjoys weightlifting and playing a variety of sports throughout the year.
