Important Coverage Information:
OHIP (Ontario Health Insurance Plan)
We direct bill your OHIP plan.
Government funded physiotherapy treatments are available to those who meet certain criteria. Ontario Ministry of Health uses an “Episode of Care” coverage system that consists of a block of physiotherapy treatments provided to patients that meet the OHIP criteria.
The number, frequency and duration of visits offered during each episode of care must be necessary and reasonable for the treatment of your condition. Your physiotherapist will use their clinical judgement and evidence-based best practices to make that decision. Keep in mind, this program does not cover services aimed to maintain an existing level of function. A new referral from a physician or nurse practitioner is required for each episode of care.
OHIP eligibility criteria:
- Age 65 or over
- Age 19 or younger
- Post Hospital stay (within in the last 12 months)
- On Ontario Disability Support Program (ODSP) or Ontario Works
- Other ODSP or Ontario Works without a health card
Employer Extended Health Plans / Benefits
We direct bill your extended health plan.
Many extended health benefits plans include physiotherapy coverage. Our team is happy to help you navigate your insurance plan coverage. While not necessary, we suggest contacting your plan provider to find out:
- If a doctor’s referral is required in order for treatment to be covered.
- How claims are handled, do they offer direct billing or will you have to complete and submit paperwork.

Motor Vehicle Accident Coverage
We direct bill your auto insurance plan.
If you have extended healthcare benefits that cover therapy, auto insurance companies in Ontario require that your claim be placed through your extended health plan first. Any remaining balances for services are then covered by your auto accident insurance plan. On your first visit, you may be asked to provide information about your extend health plan.
If you do not have an extended healthcare benefits that covers therapy, your treatments may be billed directly through your auto accident insurance company.
Workplace Safety & Insurance Board (WSIB)
We direct bill for Workplace Safety & Insurance Board (WSIB) for you.
If you are injured while at work, you have the right to choose which healthcare provider you get care from. You do not need a doctor’s referral to see a physiotherapist. For more information about your rights as an injured worker please see the Workplace Safety and Insurance Act.
If you sustain a workplace injury, you must contact your workplace immediately to ensure the proper paperwork is completed and that your claim is started.
WSIB provides physiotherapy coverage for workers injured on the job. We bill WSIB directly for treatments provided, complete necessary forms, progress reports, treatment extensions and communicate with adjudicators, your workplace and other healthcare professionals involved in your treatment. Our team can help you navigate through any WSIB paperwork that you may be unsure about.
Blue Cross Billing
We will bill Medavie Blue Cross directly for physiotherapy and massage therapy treatments for members of:
- Royal Canadian Mounted Police (RCMP)
- Department of National Defense (DND)
- Canadian Forces
- Veterans Affairs
For any treatments partially covered, you will be asked to pay the difference at each visit.

Tara Church - Physiotherapist
Tara holds an Honours B.A. from the University of Guelph and a Bachelor of Science in Physical Therapy from the University of Toronto. She has extensive experience in outpatient orthopaedics, including post operative knees, hips and shoulders. She has a special interest in postural issues. With gentle persistence, Tara encourages her patients to take a proactive role in their health and rehabilitation.
Anish Sharma - Physiotherapist Resident
Anish received his Masters in Physiotherapy from La Trobe University in Melbourne, Australia. He uses a variety of techniques which include manual therapy, joint mobilizations, and soft tissue release. He is also a firm believer on the importance of exercise in creating resilience in the human body.
Adrian Avenido - Physiotherapy Support Personnel
Adrian Avenido graduated as a physiotherapist in 1997 in the Philippines. He moved to North America in 2001 and has been working as a physiotherapy assistant ever since. Adrian has worked in many different clinical environments including hospitals, private clinics, and long-term care homes. When he’s not at work, he enjoys playing basketball, volleyball, and tennis.
Celina Jalloul - Massage Therapist
Celina holds a Bachelor of Science in Kinesiology from York University and is a Registered Massage Therapist. She has worked at a multidisciplinary clinic since graduation focusing on motor vehicle accidents and injury rehab. She has experience working with various conditions and age groups while advancing her expertise through continuous education in courses focused on neuromuscular conditions and treatment, pain management and manual therapy. Her special interests include myofascial release, acupuncture, and exercise rehab. Celina believes successful rehabilitation begins with positive self-awareness.
Wendy Rolek - Massage Therapist
Registered Massage Therapist
Wendy Rolek is a Massage Therapist practicing in Toronto, ON. She graduated from Canadian College of Massage and Hydrotherapy in 2000 with a Registered Massage Therapist.
Wendy focuses on Treats Dufferent Conditions.
Wendy Rolek graduated from the Canadian College of Massage and Hydrotherapy in North York and gained experience working in both outpatient physiotherapy and chiropractic clinics. She has acquired certifications as a Registered Massage Therapist and in Basic Shiatsu.
Johann De Chickera - Naturopathic Doctor
Johann completed his 4-year degree at the Canadian College of Naturopathic Medicine, in Toronto. His clinical focus lies in chronic disease such as those related to the Gastrointestinal, Endocrine, Immune Systems. Johann is happy to be apart of PT Health as he also has a special interest in Sports and Rehabilitation.
Being a part of competitive sports all his life, as well as being a regular gym-goer, Johann appreciates how important physical medicine is and how frustrating it can be when physical, psychological and mental ailments impede one from living life to it’s fullest potential.
His approach to medicine relies on working with the patient to come up with a feasible, multi-factorial approach that addresses all complaints at once. He employs a strong background in diagnostic medicine and human physiology and pathology to diagnose and treat. His treatments involve a combination of nutritional counselling, botanical medicine, eastern medicine (acupuncture), nutraceutical supplementation and hands on physical medicine.
Farah Mawani - Physiotherapist
Farah Mawani graduated from McMaster university in 2008. She has experience treating a wide range of orthopedic conditions. Her special interests include acupuncture, manual therapy and sports rehab. Farah believes that the best way to recover from injury is through a combination of active exercise and manual therapy. In her free time, she enjoys playing volleyball, hiking and cycling.
Dr. Scott McCrae - Chiropractor
Dr. McCrae is a graduate of both the University of Victoria (Sciences) and the
Canadian Memorial Chiropractic College here in Toronto. He practices at several pt Health clinics around the GTA.
Since finishing his studies Dr. McCrae has practiced a number of years in South America with the balance of his professional life spent practicing here in Toronto and the GTA.
Besides adding Acupuncture to his resume since graduation, Dr. McCrae has become the gait scan professional who fits and dispenses orthotics for a number of our PT Health clinics. He also does custom bracing.
